U-8 & Above: This is a Fill-In & Print form and obtain the required wet-ink signature: This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over. Players who are age 18 or over MUST sign their own form. The wet ink signatures should be in a color. Two copies of this form must be turned in to your League/Club Registrar with the appropriate fees. You may make a copy of the signed form and turn the original and the copy in or you may turn in two signed copies. Contact your League/Club Registrar if you have any questions.
